I've seen this a few times here and there, though not so often as you
seem to be seeing it. It's happened in quite a few different apps
for me that use the Unified toolbar & title look. Mail has done it
to me more often than any other, but that's probably because I use it
more often than the others and always have it running.
I did see it happen once in one of my apps (that I'm working on at
the moment and just added this option to). I'm fairly certain it's a
redraw issue somewhere in Cocoa.
